| Appointments
We welcome all patients. We see
patients Monday through Friday at offices in Arlington
Heights and Elk Grove Village.
Just call our central
number – 847-797-9000 – for
an appointment at any of our three offices.
This number can also be used to contact the
physicians, nurse practitioner, or staff at
The Center for Advanced Breast Care for any
of your questions or concerns. After business
hours and on weekends, your call will be taken
by an answering service, who will contact us
in the event of an urgent problem.
If you need to cancel
an appointment, please call at least 24 hours in
advance so we can accommodate others who need appointments.
Fees and Payment
We work with each patient to
make financial arrangements. We accept many major
health insurance plans, as well as Medicare assignment.
Bring your insurance card to your first appointment.
If your healthcare plan requires a referral from
your primary care physician, please bring the
referral slip to your first appointment. We will
prepare and file claims with your insurance company
as a courtesy. You will be responsible for paying
the balance.
Your First Appointment
Please bring all of your prior
films and other scans to your first appointment.
In order to best evaluate your case, it’s
very important for you to make arrangements to
pick up copies of all previous films, scans, and
other test results.
Forms
As with your first visit to
any new healthcare provider, there will be forms
for you to complete prior to being seen. It may
be more convenient for you to download, print,
and complete the forms prior to your appointment,
then bring them along with you. Just click on

Please call during office hours
for prescription refills.
Hospital Affiliations
and are on the medical staff at Northwest Community Hospital in Arlington Heights and at Alexian Brothers Medical Center in Elk Grove Village.
